FCP '25-'26: Welcome, Info & Paperwork

Good Afternoon FCP new and returning families,
Welcome to the 2025-2026 school year. I hope everyone is having a great summer! We are so excited to have your child enrolled in the coop this year. The first day of school is Tuesday, September 2nd for the 3/4s class and Pre-K class, and Wednesday, September 3rd for the Young 3's class. You will be receiving more information in the coming weeks about the first few weeks of the school year including the mandatory parent orientation and the meet the teacher/class popsicle party set up by the classroom teachers.
In the email send on 7/10/25 you will find the items listed and described below:
Calendar for the 2025-2026 school year: Please note two mandatory events in August: Our classroom/playground clean up and paperwork turn in on 8/18 from 5:00-8:00PM and our parent orientation on 8/27 from 6:00-8:00PM. These are both child-free events.
Tuition Letter: This document shows the tuition for both the 3-day/week class and the 4-day/week classes. You have 3 payment options: pay in full, 2-payment plan, 4-payment plan. We ask that families choose one of these three options described on the tuition letter prior to August 18th. When you come to turn in your paperwork on August 18th, I will mark down which payment plan you have selected and will send you the first tuition invoice through Quickbooks.
Enrollment Form: Must be printed, filled out legibly and thoroughly, and signed by a parent as well as by myself when we meet to go over forms. Note: If you answer "yes" to any of the yes/no questions then you must fill out a Child Medical Physical Care Plan.
Child Medical Form: Must be completed by the child's pediatrician. We also need an up to date vaccination record. This is the most time sensitive, so please request that your pediatrician fill this out ASAP. Note that the date of the child's well visit must be within the last 12 months. We will need a new medical form to be filled out during the 2025-2026 school year once the date of exam has expired (12 months from exam date).
Child Medical Physical Care Plan - Only required for medical conditions, allergies, administration of emergency medication or care, etc. The 2nd part of this form must be completed by a doctor. If this form does not apply to your child, you do not need to submit it.
Administration of Medication form: Only required if medication needs to be given during the school day.
Student Release Form - This must be printed and completed, indicating who has permission to pick up your child from school.
**RETURNING FAMILIES ONLY: If nothing has changed
You do not need to submit another Enrollment Form or Student Release Form.
You also do not need to turn in a new medical form unless your child has recently had his/her yearly well check. As long as the medical form I have on file is current then you will not need to resubmit until 12 months from the exam date listed on the form.
You will still need to come to the paperwork turn in date so that you and I can both initial your child's enrollment form indicating that it has been reviewed by both parties for this school year. You will also indicate your tuition payment option on this day.
Hard copies of these forms are due in person on Monday, August 18th from 5:00-8:00PM during our playground/classroom cleaning event. To reiterate, ALL children must have an Enrollment Form, Medical Form with Vaccination record, and Student Release Form. The Child Physical Care Plan and Administration of Medication forms are only to be completed if they apply to your child. I look forward to seeing all of you on August 18th.
